IDENTITY is a website that displays up-to-date health information from the electronic health record matched with child welfare information from the Ohio Statewide Automated Child Welfare information System for children currently in Hamilton County and Montgomery County custody.
IDENTITY displays up-to-date custody status, caseworker and current placement contact information, and detailed child welfare and health history, including current case information, maltreatment history, abuse and neglect reports, health information, and referrals and appointments.
