If you are 16 or older and are in the custody of Hamilton County Job and Family Services, then you may have received an I Care Guide through one of the people on your support team (ex. your doctor, social worker, independent living specialist). The I Care Guide is your go-to source for a summary of your personal health information. With an up-to-date guide, you can easily find your medications, shot records, and important medical contact information. Tools like the STI Symptoms Checklist and Healthcare Access Decision Aid are also included in the guide to help you be in control of your healthcare.
